What do I need to make a first-time appointment?
     Basic demographics of the client (name, address, email address, phone numbers, social security numbers, date of birth), as well as your insurance card
information. Your consent for treatment and approval to submit a claim to your health insurance on your behalf is require before services can be
rendered.

Do I have to have a referral from a doctor in order to make an appointment?
     No. You may call anytime and make an appointment without a referral.

Can I call for my child and make an appointment for them?
     Yes, but once the client is 18 years old or older, they must call to make their own appointment and provide the necessary information. We cannot make
an appointment for anyone 18 or older without their consent.

Does anyone in your office prescribe medications?
     No, we currently do not have any clinicians on staff that can prescribe medications.

Do you take my insurance?
     Our office accepts most insurance plans, including Medicaid and Medicare. Please talk to our front office staff as they can help determine if your
insurance will cover a portion of your visits. Clients are encouraged to check that a particular therapist is covered in-network.

What if I am a self-pay client?
     Your first visit will be $150.00, and each additional visit will be $115.00. We secure a credit or debit card during registration and those payments are
expected at the time of your visit.
